Dahlonega Retreat Waiver

Waiver: I understand that I am participating in yoga classes, health programs, workshops and/or other wellness, body work, therapy, exercise and healing arts activities (collectively, the “Activities”) offered by Sonya Phillips and Stacy Mathews. I recognize that said activities require physical exertion which may be strenuous and may cause physical injury and I am fully aware of the risks and hazards involved. I understand it is my responsibility to consult with a physician prior to and regarding my participation in said activities. I represent and warrant that I am physically fit and have no medical condition which would prevent my full participation in said activities. The following are contraindications/limitations for said activities:  pregnancy, glaucoma, recent surgery (esp. shoulder, hip, eyes, back, hands or wrists), heart disease, very high or very low blood pressure, easy onset vertigo, osteoporosis/bone weakness, recent head injury, propensity for fainting, cerebral sclerosis, carpal tunnel syndrome, severe arthritis, disc herniation, muscle spasms, recent stroke, botox injections (within 6 hours). I knowingly, voluntarily and expressly waive any claim I may have against Sonya Phillips and/or Stacy Mathews for injury or damages that I may sustain as a result of participating in said activities. I, my heirs or legal representatives forever release, waive, discharge and covenant not to take any legal action against Sonya Phillips and/or Stacy Mathews for any injury or death caused by own negligence or other acts.

I acknowledge that I have carefully read this agreement and fully understand its contents. I voluntarily and knowingly agree to the terms and conditions stated herein. I am aware that by clicking the acknowledgment box below, I am giving up substantial rights, including my right to sue and certain legal rights my heirs, next of kin, executors, administrators and assigns may have against any Released Party.

I understand that I will pay a minimum $400 deposit to reserve my spot by August 1st, 2023 of which $100 is non-refundable. The remaining balance is due on or before October 1st, 2023. Forfeiture of $400 deposit if cancellation occurs between October 2nd – October 20th. If reservation is cancelled after October 20th, all funds are non-refundable.
